[Mathregionals] INFORMATION REQUIRED - Worksheet for Provost

Hi all,

The provost's office has tasked us with filling out a worksheet to help them track readiness for Monday.  This is beyond my ability to fill in alone, so I will try to divide the labor.

If this doesn't work for you (and it may not, as I've not tried this before), then please get me the information by replying to this email with your responses to the following for each of your courses.
