[ITSA] 2011-12 Officer Nomination Form

Hi all,

Here is the nomination form for the 2011-12 ITSA officers. This form will be open until May 9th, giving us just about a week to take nominations. Feel free to nominate yourself or anyone you feel would do a good job. Multiple nominations are also acceptable, so you can nominate more than one person per office. So, please take a couple seconds to help make our program even better!
