Dear Participant,

We are inviting you to participate in a survey investigating whether a relationship exists between anxiety and academic self-efficacy in graduate students who face personal academic challenges in CACREP-accredited programs. The survey is a part of the research study titled “Challenges for Graduate Students’ Academic Success.” The study is being conducted by researchers at Governors State University in Illinois by Sakina Hull and Dr. Kent B. Provost (PI). We invite you to participate because you meet the inclusion criteria for the study. You are:

At least 18 years old.

Currently enrolled in a CACREP-accredited master’s or doctoral-level program.



The survey should take between 10 to 15 minutes to complete. You will be asked:

Focus and organization: e.g., “How often do you have difficulty getting things in order when you have to complete a task that requires organization?”

Stress and anxiety: e.g., “How often have you been bothered by feeling nervous, anxious, or on edge?”

Academic performance confidence: e.g., “How much confidence do you have about participating in a class discussion?”



Your basic demographic information will also be collected.



Participation in the survey is voluntary, and you may refuse to participate or exit the survey at any time. You may also skip any questions you do not want to answer.



Participation in the survey will not benefit you directly, but it may help researchers expand the understanding of academic challenges faced by graduate students and offer insights that could inform counselor educator training and the growth and development of counselor trainees. Additionally, I hope this study will stimulate discussions on the need for additional resources to support students in these programs facing such challenges.



The risk of participation in this survey is minimal, however, you should be aware that there is always a possibility of security breaches of data collected via the internet. We will make every effort to protect your responses once the data is downloaded from the internet. The data will be stored on a secure server at GSU and will not be shared with anyone beyond the research team.
