 This is exactly what is covered in “Creating Module Surveys”  it will allow you to create a “booklet” questionnaire (which may be generated for Paper or for Online respondents).

I have attached a sample of the Questionnaire we will be creating.  This is the paper version, but I demonstrate both paper and online surveys in the webinar.
