 This is exactly what is covered in "Creating Module Surveys"  it will allow you to create a "booklet" questionnaire (which may be generated for Paper or for Online respondents).

I have attached a sample of the Questionnaire we will be creating.  This is the paper version, but I demonstrate both paper and online surveys in the webinar.

Micheal,

I have been looking over your upcoming Webinars, and am wondering if one deals specifically with the following issue.

We have team-taught courses that are still using paper evaluations for each instructor, in order to evaluate them individuallly.  Additionally, students are asked to complete a Class Climate survey to evaluate the course overall.

I'm wondering how the online evaluations can be redesigned for multiple instructors, without sending out separate evaluations to students for each instructor of a team-taught course.

Please let me know if there is a good way to do this - maybe it's in the CC Users Guide and I'm just missing where that is.
