This is your final reminder that the OPERS exemption request forms are due by the end of this month. If you have not done so already, please follow the link towards the bottom of this e-mail that says "Request for: Optional Exemption as a Student." Please print off this form and fill out only the top half of the form. You do not need to know your employee ID, so don't worry about looking that up, just get the form filled out and turned in ASAP if you have not done so already.

SUBJECT:         Filing for Exemption from Retirement Contributions

You are receiving this message because you have a current active appointment as a student employee at Ohio University.  Effective October 2013, the Ohio Public Employees Retirement System (OPERS) revised the Request for: Optional Exemption as a Student<https://www.opers.org/forms-archive/F-3.pdf> form (Form F-3) and the Request for Optional Exemption as a Student Instructions<https://www.opers.org/forms-archive/F-3_Instructions.pdf>. If you wish to be exempt from making contributions to retirement on your student salary[1], you need to submit form F-3 within your first month of employment.  This form needs to be completed and submitted to the Payroll Office within 30 days or retirement contributions will be withheld from your paycheck.  NOTE:  it is possible that your supervisor has already requested this form from you this semester.  If you have already submitted it, you do not need to do so again.

Only those students who meet the minimum requirements for enrollment are eligible for exemption.   The minimum requirements are enrollment in at least six (6) credit hours for undergraduate students and five (5) credit hours for graduate students.  By completing this form, students are requesting an exemption from required employee contribution to the OPERS pension system, which is currently 10% of earnable salary.  Also, pension service credit will not be earned for applicable student employment.  If a student later becomes a member of OPERS, the student may be eligible to purchase this exempt service per Ohio Revised Code section 145.28.

To request exemption:


*       Complete Step 1 of the Request for: Optional Exemption as a Student<https://www.opers.org/forms-archive/F-3.pdf> form,
This form requires your Social Security Number. Your Employee Number is optional and can be found at: http://eelookup.admsrv.ohio.edu/eelookup/

*       Return the completed form to the Payroll Office either via campus mail (Payroll Office - WUSOC 214) or US Mail (Ohio University Payroll Office; WUSOC 214; 1 Ohio University; Athens, OH 45701)

*       Ohio University's Payroll Office will complete the Step 2: Employer Authorization portion of the form and submit it to OPERS.

Please review the form instructions regarding completion of the form.  Remember that prior versions of the form will not be accepted by OPERS; you must submit the 10/2013 version of the form.
