<p class="MsoNormal">I am in the process of working on an application for Kentucky, this application would be for
<b>CLINICAL/ INTERNSHIP/FIELD EXPERIENCES</b>. This would be for those students who just complete
<u>all field experiences</u> in KY and do the rest of their education face-to-face outside of Kentucky on a campus where the institution is located in another state.
<b>This is not for ONLINE/distance education students.</b><o:p></o:p></p>
<p class="MsoNormal"><o:p> </o:p></p>
<p class="MsoNormal">With the application there will be a <u>$5,000 base fee plus offering fees ($200 per internship/ clinical/ practicum course)</u>. Please note that if this particular application is approved, it will not be for the entire University; the
only courses that will be authorized in this state will be those that are listed on the application, invoiced and paid for.<o:p></o:p></p>

<p class="MsoNormal">If you are interested in this opportunity please contact me by
<b>April 17, 2015</b>. I must stress to all interested parties that there will be program/course specific information that will be required with this application; I will need your cooperation in order to get this information/paper work returned to me by
the requested dates or earlier in order to get this application approved in a timely manner. Also, the application will not be reviewed until the invoice from KY is paid and this process
<i>could</i> take up to 6 months for approval. <o:p></o:p></p>
