placement for which you are receiving supervision. The study will take about 10-15 minutes of your time.<o:p></o:p></span></p>

<span style="font-size:9.5pt;color:black">We will protect your confidentiality by storing your data securely on a password protected, encrypted computer. Only my colleagues and I will have access to the data. The data will be anonymous as you will not be asked
for any identifying information. IP addresses will be not tracked or recorded as a part of this research. Only summarized data will be presented at meetings or in publications.</span><span style="font-size:9.5pt"><o:p></o:p></span></p>

randomly once the study is concluded. IRB review has determined that risks to participants are minimal. The chief risk is that some of the questions may make you uncomfortable. You may skip any questions you do not want to answer. Your participation is voluntary,
