Dear Dr. Bhat,
My name is Madeleine Stevens, a graduate student within the CACREP-accredited counseling program at Youngstown State University (YSU). This email is meant to request permission from the director and/or chair of your Master's-level, CACREP-accredited counseling program in Ohio to study your students attitudes and beliefs about the benefits of engaging in personal counseling. If I have contacted the wrong person in your program, please provide the correct contact!

This study has been approved by YSU's Institutional Review Board -- see attached for the proposed survey. Should you grant permission, I will email faculty in your program asking that they forward a Qualtrics survey link to students. The attached informed consent document will be presented once students click on the link. Please respond directly to me with a 'yes' if you will permit me to recruit participants from your program. Please respond with a 'no' if you choose to abstain.

I am eager to learn more about the topic in question and am hoping you will assist me in my data collection.
