House of New Hope is looking for an energetic, socially intelligent, detail-oriented licensed mental health professional to join our Treatment Foster Care Therapeutic Case Management Team located out of our Licking County Office.



Job Purpose
Serves to ensure the safety and well-being of treatment foster families and children placed in the foster care system through an on-going assessment of needs; design and implementation of individualized service plans; and periodic monitoring of outcomes as specified in OAC rule and agency policy. 
Provides specific, measurable, and individualized Community Psychiatric Supportive Treatment (CPST) services and/or Behavioral Health Counseling and Therapy Services to each eligible person served with a focus on the individual’s ability to succeed in the community; to identify and access needed services; and to show improvement in school, work and family and integration and contributions within the community.



Essential Functions

Perform all mandated case management responsibilities pursuant to OAC 5101:2 and county service contracts including, but not limited to: foster child and foster parent visitations; essential periodic case documentation; and participation in case planning activities.
Coordinate the development, implementation and monitoring of the ISP and all treatment team activities and/or brokered services; including medical (EPSDT), mental health and educational requirements.
Meet or exceed established Quality Improvement performance expectations.
Provide support to foster families and foster children through the provision of on-call crisis response responsibilities, training, education, guidance, physical presence, and the promotion of trauma-informed treatment.
Advocate for consumers and stakeholders pursuant to the ISP and established case plan, assist in HONH recruitment efforts, and promote the positive reputation of the agency.
Represent HONH by acting as a liaison to social service agencies, PCSAs, and state level departments as necessary.
Perform all mandated community mental health services responsibilities pursuant to OAC 5122 including, but not limited to mental health assessments, community psychiatric supportive treatment, behavioral health counseling and therapy, and all subsequent mandated documentation requirements.
