We have created and distributed this survey in order to assess hunger and food insecurity on OU's campus. Once you have completed the survey, you will have the opportunity to enter your email address to enter a raffle for the chance to win a $25 gift card from Casa Nueva. Your email address will not be associated with any of your responses to this survey.
 
Your responses will help us determine the need for developing a food pantry on campus. All responses will remain confidential, no personally identifying information will be collected or reported, and confidentiality will be maintained by keeping aggregate data collected on in a safe and secure location, only accessible to the investigators.
 
Participation is completely voluntary, and you may withdraw your consent and discontinue at any time without any consequences or penalty. You are not waiving any legal claims, rights, or remedies by participating.
