                        Visits will begin after the program
        Event is free! You must register to attend.
2012 DD Legislative Advocacy Day
Complete a Registration Form today!
Use the form below and return by email, or print and return one of the attached forms.
        
After you register, AXIS will schedule your legislative visit and will let you know 
the time and place. Many people will be making visits, so plan to stay in Columbus 
for several hours. Bring a lunch or visit one of the nearby sandwich shops or restaurants.
Some funding is available, if needed, to assist with your transportation and attendants.

Legal Rights Service, Autism Society of Ohio, Ohio Association of County Boards, and
Ohio Statewide Independent Living Council.

REIMBURSEMENT

Checks will be mailed to the name and address at the top of the form. If you must have the money before the event, or if you require funds for an item not listed, contact AXIS to make arrangements.
