Mona, thanks for your follow up.  After talking with Jay, we made some
changes to the Call for Proposal's.  I have attached the new, hopefully
easier form, for the 2009 conference.

If you have any questions or concerns, please let me know.  If you have
started the proposal on the previous form please feel free to send that
form and I can transfer it over.

You can also share this with others who may be appropriate speakers at the
conference.
