[Classclimate-users] Printed course eval does not show course number

I've got the course numbers, instructor, and term, I'm seeing the same bar code on all of mine. I'll work on it tomorrow.

The printed course evaluation that I ran does not show the course number ... just the Instructor last name and 2131.  (See attached).
Not sure what I did ... or not...

But I sure would like to have that 'feature' turned on.
